If you are in need of equipment for your child to play a sport (or for general recreational purpose) please fill out our online Equipment Request Form or download a PDF Version and we will get back to you as soon as we can.
If using the PDF Version, please print out and complete all fields in the form. Completed Equipment Request Forms can be email to: gearupkidsfoundation@gmail.com or dropped off in person to The Brick Stettler during regular business hours for our review.
